Eliot Community Human Services operates high-quality residential and independent support services programs for adults with developmental disabilities and brain injuries. We utilize a person-centered, strengths-based approach to provide quality supports to adults of all ages.

We are seeking an experienced Licensed Practical Nurse (LPN) to coordinate residential services supporting these adults within the communities of Burlington, Haverhill, and Framingham.

This full-time position offers a competitive salary of $62,680 per year, with a schedule of Monday through Friday, 8am-4pm or 9am-5pm.

Job Description
  • Provide education and consultation on medical issues, treatments, medications, wellness, and prevention to individuals and staff.
  • Offer one-on-one skills training in specific individualized need areas and group training for individuals and staff.

The ideal candidate will have three years of experience in a community health setting, with expertise in medical administration and communication skills. A bachelor's degree in nursing is preferred, along with RN licensure.

Responsibilities
  1. Review medical documentation, complete medical assessments, and prepare medical documentation for new referrals and discharges.
  2. Participate in transition planning and coordination of medical and nursing care/services as needed for new referrals and current case loads.
  3. Hire, train, and supervise nursing staff for the Brain Injury programs.
