Eliot Community Human Services operates high-quality residential and independent support services programs for adults with developmental disabilities and brain injuries.

We utilize a person-centered, strengths-based approach that provides quality supports to adults of all ages, working closely with families and community organizations to build partnerships for growth and development.

Our team is seeking an experienced LPN to coordinate residential services in the Burlington, Haverhill, and Framingham communities.

To be successful, candidates will have 3 years of related experience and skills to establish supportive relationships with individuals with developmental disability and brain injury, while committing to meeting individual needs and preferences.

Responsibilities:
  • Provide education and consultation on medical issues, treatments, medications, wellness, and prevention.
  • Conduct one-on-one skills training in specific individualized need areas and group training for individuals and staff.
  • Offer oversight, guidance, and consultation on medical protocols, medication management, and care coordination.
  • Review medical documentation, complete medical assessments, and prepare documentation for new referrals and discharges.
  • Participate in transition planning and coordination of medical and nursing care/services as needed.
  • Perform direct nursing care and medical support services as needed.
  • Hire, train, and supervise nursing staff for Brain Injury programs.
  • Meet with managers and staff when an individual receives a new medical diagnosis or prescribed treatment, providing training and consultation.
  • Develop working relationships with community medical providers and VNA services for coordinated care.

Requirements:
  • Expert communication and computer software skills.
  • Excellent medication administration skills.
  • Bachelor's degree in Nursing (preferred).
  • Registered Nurse (RN) licensure (preferred).
  • Three years of experience in a community health setting.

This is a full-time position, Monday through Friday, with a salary range of $62,680.00 annually.
