Patient Care Advocate

This Medical Social Worker position is a key role in our interdisciplinary team, responsible for providing social work services to patients in their place of residence.
Main Responsibilities:- Consult with physicians and other healthcare professionals to develop individualized plans of care.
- Conduct social assessments to identify patients' social and emotional needs.
- Develop and implement interventions to address patients' social and emotional needs.
- Document all patient interactions and progress notes according to agency policy.
- A master's or doctoral degree from a school of social work accredited by the Council on Social Work Education, and one year of social work experience in a healthcare setting, or
- A baccalaureate degree in social work, psychology, sociology, or other field related to social work, and at least one year of social work experience in a health care setting (functions as an assistant).
- Licensed as an Independent Clinical Social Worker in the State of Washington.
- One year of experience in home care, preferred.
$65,000 - $80,000 per annum, depending on experience.
