Job Description

">

We are seeking a skilled Nurse Care Coordinator to join our team at Swedish First Hill. As a key member of our care management team, you will work collaboratively with inter-disciplinary teams to improve patient care through effective utilization and monitoring of healthcare resources.

">

Key Responsibilities

">

• Coordinate the care and service of selected patient populations through the acute care episode, across the continuum.

">

• Work collaboratively with internal and external teams to ensure seamless care coordination.

">

• Assess patients' goals, biophysical, psychosocial, environmental, economic/financial, and discharge planning needs.

">

• Procure services and resources for identified patients and families, advocating for their needs as necessary.

">

• Identify and report cases and problems appropriate for secondary review to Case Management Department leadership.

">

Required Qualifications

">

• Bachelor's Degree in Nursing or related field.

">

• 3 years registered nursing experience in an acute care hospital.

">

• Washington Registered Nurse License upon hire.

">
