Job Title: Licensed Practical Nurse LPN

Job Summary:

The Licensed Practical Nurse LPN will be responsible for providing high-quality patient care in a fast-paced healthcare environment. This individual will work closely with the healthcare team to ensure that patients receive the best possible care.

Key Responsibilities:

  • Assess and Plan: Conduct initial and ongoing assessments of patients to identify their physical, emotional, and social needs. Develop and implement individualized care plans to meet these needs.
  • Implement and Evaluate: Implement the care plan by providing direct patient care, administering medications, and performing various medical procedures. Evaluate the effectiveness of the care plan and make adjustments as needed.
  • Communicate Effectively: Communicate effectively with patients, families, and healthcare team members to ensure that patients receive comprehensive care.
  • Maintain a Safe Environment: Maintain a safe and clean environment for patients by following infection control procedures and reporting any hazards or concerns to the supervisor.
  • Document Patient Information: Accurately document patient information, including medical history, treatment plans, and progress notes.
  • Collaborate with Healthcare Team: Collaborate with the healthcare team to ensure that patients receive comprehensive care. Participate in interdisciplinary meetings and contribute to the development of patient care plans.
  • Provide Education and Support: Provide education and support to patients and families to promote health and wellness.
  • Perform Administrative Tasks: Perform administrative tasks, such as charting, ordering supplies, and maintaining patient records.

Requirements:

  • Education: Graduate of an approved LPN program.
  • Experience: Minimum of 1 year of experience as an LPN in a healthcare setting.
  • Skills: Strong communication and interpersonal skills, ability to work effectively in a team environment, and ability to maintain confidentiality.
  • Certifications: Current LPN licensure in the state of employment.

Working Conditions:

  • Works in a fast-paced healthcare environment.
  • Subject to frequent interruptions and changes in priorities.
  • May be required to work beyond normal working hours, including evenings, weekends, and holidays.
