Job Summary
The Licensed Practical Nurse (LPN) in our Home Health department plays a vital role in delivering high-quality patient care in the comfort of patients' homes. As a member of our interdisciplinary team, the LPN will work closely with patients, families, and healthcare professionals to develop and implement personalized care plans that meet the unique needs of each patient.
Key Responsibilities
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Collaborate with the interdisciplinary team to develop and implement patient care plans that address physical, emotional, and social needs.
- Conduct thorough assessments of patients' conditions, identifying areas for improvement and developing strategies to address them.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
- 1 year of experience in a nursing-related field (preferably in home health or a similar setting).
- Graduate of a Practical Nursing program.
- Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
Work Schedule
Monday through Friday, 8:00 AM to 4:30 PM. Occasional weekend and evening shifts may be required.
