We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Clinical Care Coordinator in our Ambulatory Clinic. As a key member of our multidisciplinary team, you will play a vital role in coordinating patient care, ensuring seamless transitions, and promoting quality outcomes.
Key Responsibilities- Coordinate patient care across various settings, including ambulatory clinics, hospitals, and community resources.
- Develop and implement individualized care plans, ensuring alignment with patient goals and preferences.
- Collaborate with healthcare teams to ensure timely and effective communication, reducing fragmentation and improving patient satisfaction.
- Identify and address potential safety concerns, medical errors, and near misses, promoting a culture of safety and quality.
- Provide education and support to patients and families, empowering them to take an active role in their care.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- Current RN licensure in New Mexico or eligibility for reciprocal agreement.
- 2 years of direct experience in a clinical setting, preferably in ambulatory care or case management.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Bilingual English/Spanish proficiency, with the ability to communicate effectively with patients and families from diverse linguistic and cultural backgrounds.
