Intake Placement Coordinator

We are seeking a highly skilled and compassionate Intake Placement Coordinator to join our team at Upbring. As a key member of our child welfare team, you will play a critical role in ensuring the smooth intake and placement process for youth in our care.
Key Responsibilities- Intake Process Management: Manage and complete the intake process for every youth entering our programs, including completing initial paperwork and transporting minors to foster homes.
- Placement Coordination: Coordinate the placement of minors with third-party transport companies and ensure timely and safe transportation to foster homes.
- Case Management: Complete the Preliminary Service Plan (PSP) within 72 hours of youth placement and coordinate any identified immediate needs through community-based services.
- Communication and Collaboration: Prepare and maintain data and information for assigned youth and services provided, complete reports as needed, and update youth data on the ORR UAC Portal.
- Teamwork and Support: Assist other staff as needed with child discharges, transport minors to required services, and work closely with management and the team to ensure effective and efficient case management and program services.
- Education: Bachelor's degree in Social Work, Human Services, or a closely related field.
- Experience: 1 year of relevant experience in child welfare or a related field.
- Skills: Strong interpersonal skills, ability to establish rapport and trust with children, parents, and families, and proficiency in Microsoft Office applications.
