Activities Care Coordinator

The Bassett Healthcare Network is committed to delivering high-quality patient care, and we're seeking a skilled Geriatric Activities Assistant to join our team.
Key Responsibilities:
- Assist the Activities Director in planning and implementing group and individual activities for residents.
- Take daily attendance of group and individual resident contact, maintaining accurate records.
- Maintain, sort, and distribute materials, and set up areas for activities.
- Perform Initial Assessments on new admissions to ensure personalized care.
- Regularly consult with the Activities Director to discuss program ideas and resident involvement.
- Assist in compiling lists for specific residents and devising group and individual programs.
- Ensure areas are clean and organized, adhering to state regulations.
- Prepare progress notes and maintain accurate records.
- Maintain confidentiality and adhere to HIPAA regulations.
Requirements:
- 6 months experience working with geriatric patients or in activities-related fields.
- Current NYS Nurse Aide Certification and valid NYS Driver's License required.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently and as part of a team.
- Medium work, occasional lifting, and bending required.
