Job Overview

Liberty Home Care is seeking a highly organized and detail-oriented Care Coordinator to join our team. In this role, you will be responsible for planning and scheduling patient visitations and tasks for our Personal Care Aide (PCA) and Home Health Aide (HHA) staff.

Key Responsibilities:
  • Develop and maintain daily staff schedules to ensure seamless patient care
  • Coordinate services with contracted agencies to meet patient needs
  • Communicate effectively with staff regarding job assignments, requirements, and concerns
  • Prepare and maintain accurate reports, including weekly referral reports and staffing needs
  • Verify time cards and identify variances for payroll and billing purposes
  • Process requests for home care services and referrals
  • Maintain confidentiality of patient information
Requirements:
  • Associate's degree or higher in a related field, or equivalent work experience
  • Strong verbal and written communication skills
  • Excellent organizational and time management skills
  • Computer skills, including Microsoft Windows and Office products
  • Ability to work under pressure and maintain a high level of productivity

Working Hours: Monday - Friday, 8:30AM - 4:30PM

On-Call Rotation: Willingness to accept on-call assignments when necessary
