Client Care Coordinator-Homecare Specialist

Visiting Health Services of New Jersey (VHSNJ) provides comprehensive home health care and hospice services to patients and families in Passaic, Bergen, and parts of Morris counties. Our multidisciplinary clinical and support teams provide high-quality care to patients in their homes.

Responsibilities:- Schedule visits following our standardized process, ensuring that only physician-ordered visits are scheduled. Notify supervisors when requests require a physician order. Update client schedules daily to ensure they are current according to our policy and patient needs.
- Verify authorizations and notify clinical supervisors of activities that require direction or input. Track and maintain disaster plans as needed.
- Validate caregiver documentation against schedules and reconcile any discrepancies. Notify supervisors of outstanding documentation and interface with payroll and billing processes.
- Recognize patient grievances and direct them to the appropriate manager.
- Assist the On-call RN with weekend and/or daily on-call processes. Schedule weekend visits/SOCs in a timely manner and enter them into the appropriate system.
- Ensure that every assigned SOC has been scheduled in the appropriate system.
- Answer the telephone promptly and in a courteous manner. Take accurate and complete messages and relay them to the appropriate party.
- Maintain client/employee schedules accurately. Enter them into the appropriate system in a timely manner.
- Fax transfer forms to facilities and coordinators within 24 hours.
- Perform other duties and/or projects as assigned. Demonstrate flexibility with job responsibilities in all areas.
- Adhere to HMH's Organizational competencies and standards of behavior.
- Lift a minimum of 10 lbs., push and pull a minimum of 10 lbs., and stand a minimum of 2 hours a day.
- High School diploma, general equivalency diploma (GED), and/or GED equivalent programs. Associate's Degree preferred.
- Two (2) Years Customer Service Experience
- Proficient in Microsoft Office Suite
- Strong Organizational Skills
