Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team as a Medical Care Coordinator. In this role, you will be responsible for coordinating medical and psychiatric care for residents, ensuring proper documentation and regulatory compliance. You will also assist with hands-on training of residential staff to maintain ongoing compliance.
Key Responsibilities- Coordinate medical and psychiatric care for residents, including medication administration and documentation
- Assist with training of residential staff on medical and psychiatric care
- Identify and organize appointments for residents, including medical and psychiatric care
- Transport and accompany residents to medical appointments, completing follow-up documentation as needed
- Perform delegated nursing tasks as directed by the RN and under supervision
- Oversee medication administration, including verification of prescriptions and ordering of medications
- Maintain medication inventory, including prescription medications and over-the-counter medications
- Ensure maintenance of medically related documentation, including MARs and MD Consults
- Update RN daily on resident medical status
- Complete documentation prior to end of shift and report issues/concerns to RN and Residence Manager
- New York State License Nurse
- Specialized training in I/DD and/or one year experience in community health care services
- Excellent verbal and written communication skills, including strong organizational, detail, and interpersonal skills
- Strong team building skills and ability to work effectively with diverse populations
- CPR and First Aid Trained
