Position Overview


The Intensive Community Wrap Around Supports program collaborates with the Department of Developmental Services and the Department of Mental Health to provide comprehensive, in-home and community-based services for adults aged 18 to 30 who are diagnosed with Autism Spectrum Disorder and Serious Mental Illness, or other significant mental health challenges.

The primary objective of this initiative is to ensure individual stability and effective functioning within their homes and communities. This is achieved by delivering clinical services and coaching support that identifies and addresses barriers to stability while fostering individual progress and skill development.


Key Responsibilities
The Registered Nurse will be tasked with conducting psychiatric evaluations, assessing physical health requirements, making suitable referrals to community healthcare providers, and overseeing medication management and administration.

Collaboration with the multidisciplinary team, individuals receiving services, and their support networks is essential to develop and update comprehensive assessments and tailored treatment plans.


Additional Responsibilities Include:
- Managing pharmaceuticals and medical supplies.


- Participating in, leading, or contributing to the creation, documentation, implementation, evaluation, and modification of comprehensive assessments and individualized treatment plans in partnership with the multidisciplinary team and individuals served.


- Offering clinical and non-clinical support through psychoeducation for individuals and families, educational and vocational counseling, assistance with Activities of Daily Living (ADLs), ensuring safe and stable housing, teaching life skills, and managing symptoms.

- Engaging in on-call rotation as required.

Qualifications
- Registered Nurse license in Massachusetts is mandatory.

- A minimum of 3 years of experience working with individuals facing psychiatric challenges, ASD, and other developmental issues is required.

- A valid Driver's License, a clean driving record, and reliable transportation are essential.
