As a Registered Nurse Case Manager, you will play a pivotal role in coordinating client care and serving as the primary contact for all healthcare disciplines involved in patient treatment. You will oversee the frequency of visits throughout the care episode.
Your responsibilities will include consulting with physicians as necessary, providing updates on patient care, and ensuring accurate OASIS data collection. Additionally, you will maintain current medication profiles, ensure timely reporting of lab values to physicians, and participate in weekly case conferences and monthly team meetings.
This part-time salaried position requires a commitment of 24 hours per week, distributed over four days, and is eligible for our paid days off program.
To qualify for this role, Registered Nurses (RNs) must meet the following criteria:
- Possess a current Registered Nurse (RN) license in the state of employment
- Have a minimum of one year of clinical experience (preferred)
- Demonstrate proficiency in contemporary nursing practices
- Hold a valid state driver's license and automobile liability insurance
