Role Overview:

The Nurse Practitioner at Oak Street Health is responsible for providing exceptional primary care to Medicare patients, focusing on enhancing their health and minimizing hospital admissions. Nurse Practitioners operate both independently and in collaboration with physicians to ensure holistic patient care. Establishing strong relationships with patients is essential in this role, fostering trust and clarity regarding treatment plans.

Nurse Practitioners engage with a multidisciplinary healthcare team in outpatient environments, emphasizing primary care, care coordination, and population health management. Collaborating with specialists and hospitals ensures access to specialized and acute care when necessary.

Key Responsibilities:
  • Deliver outstanding primary care services
  • Conduct routine and urgent office consultations
  • Perform wellness examinations and health risk evaluations
  • Coordinate care with other healthcare providers and facilities
  • Lead initiatives in population health
  • Support phone triage and patient education efforts
  • Participate in community outreach activities
  • Conduct home visits as needed
  • Additional responsibilities as assigned
This role reports to the Center Medical Director and works closely with operational leaders.

Essential Qualifications:
  • Master of Science in Nursing or equivalent qualification
  • National certification in a relevant specialty area
  • Active Nurse Practitioner license in the state
  • Active DEA registration
  • Authorization to work in the US
  • Preferred experience in primary care
  • Familiarity with health documentation and coding practices
  • Collaborative team player with a focus on population health
  • Bilingual skills are advantageous
