Position Overview:
The advanced practice provider (APP), either a Nurse Practitioner or Physician Assistant, plays a vital role within the acute care hospital environment, collaborating closely with a multidisciplinary healthcare team. This position focuses on delivering comprehensive care within the Nephrology Transplant Division of the Department of Medicine.
The Acute Care Nurse Practitioner will be responsible for managing the care of patients who have undergone kidney or pancreas transplant surgeries, in both outpatient and inpatient settings. In the outpatient clinic, this role includes overseeing the care of patients on the transplant waiting list and providing follow-up care for transplant recipients. Additionally, the Nurse Practitioner will offer consultation services and participate in hospital rounds alongside physicians and fellows.
Key Responsibilities:
- Conduct thorough patient histories and physical examinations, documenting findings in the medical record.
- Utilize appropriate assessment techniques to gather relevant diagnostic information and perform necessary procedures.
- Evaluate risks associated with managing acute and complex chronic conditions, including medication side effects and fluid imbalances.
Diagnosis and Management:
- Collaborate with the healthcare team to diagnose acute and chronic health conditions.
- Manage diagnostic testing, including ordering and interpreting results.
- Develop differential diagnoses and address complications with appropriate interventions.
Care Planning:
- Identify expected outcomes and formulate a comprehensive plan of care tailored to complex health needs.
- Implement evidence-based practice guidelines to create a dynamic care plan applicable across various settings.
- Modify care plans as necessary and prescribe therapeutic interventions.
- Perform advanced procedures as permitted by qualifications and competency.
Communication and Collaboration:
- Maintain effective communication with the interdisciplinary healthcare team.
- Facilitate continuity of care through collaboration with patients, families, and staff.
Documentation:
- Document key components of patient progress through timely notes and summaries.
- Ensure documentation meets compliance standards and accurately reflects patient acuity.
Professional Development:
- Exhibit professional behaviors, including mentoring and educating graduate students.
- Engage in research opportunities and analyze evidence-based practices.
- Maintain continuing education units and professional organization memberships.
Qualifications:
- Master's Degree in a relevant field.
- National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant.
- BLS certification.
