Job Overview:

This full-time role involves providing essential support to patients and their families within the community. As a Healthcare Social Services Coordinator, you will engage with individuals in their own environments, offering assistance and guidance during challenging times. Your work will take place across multiple locations, allowing for a dynamic and flexible work experience.

As a vital member of a collaborative care team, which includes social workers, healthcare professionals, and support staff, you will focus on education, resource facilitation, and advocacy. Your efforts will be directed towards enhancing the quality of life for patients and their families.

Key Responsibilities:

  • Deliver social services to patients and families in their residences.
  • Work alongside a multidisciplinary team to ensure personalized care aligned with each patient's needs.
  • Provide educational resources and advocate for patients and their families.
  • Participate in on-call duties and after-hours support as necessary.

Qualifications:

  • Bachelor's Degree in Social Work (BSW) is required; a Master's Degree in Social Work (MSW) with appropriate licensure is preferred.
  • At least one year of supervised experience in a healthcare setting or relevant internship.
  • Experience with elderly populations is advantageous.
  • Excellent interpersonal and communication abilities.
  • Willingness to travel between various locations as part of the role.
