Position Overview:
The Nutrition Specialist evaluates and monitors the nutritional health of participants in the Senior Community Care program. Utilizing relevant data, the specialist formulates and executes suitable nutritional strategies while collaborating with the Interdisciplinary Team to fulfill the dietary needs of participants. This role includes conducting nutritional assessments and providing educational resources for participants requiring dietary interventions. The specialist also oversees meal service coordination and ensures compliance with established standards in kitchen operations.
Qualifications: To excel in this role, candidates must demonstrate the ability to perform essential responsibilities effectively. The following qualifications are essential:
Mandatory Requirements:
- Bachelor's or higher degree in food and nutrition or dietetics from an accredited institution
- State licensure as a Dietitian and Registered Dietitian Certification
- At least one year of experience working with elderly or frail populations
- Successful completion of a Background Check and Drug Screening
- Medical clearance for communicable diseases and up-to-date immunizations prior to participant interaction
Core Responsibilities:
- Deliver high-quality Nutrition Care following the Academy of Nutrition and Dietetics standards, including conducting Nutrition Assessments, Diagnoses, Interventions, and ongoing Monitoring and Evaluation. Provide Medical Nutrition Therapy and education to participants and caregivers, reporting any changes in participant health to the Interdisciplinary Team.
- Conduct annual in-person re-assessments and additional assessments as needed based on participant status changes.
- Actively participate as a member of the Interdisciplinary Team, attending meetings to discuss participant changes and collaborate on care planning.
- Monitor participants' weights, medical conditions, medication interactions, and overall health, reporting any changes to the Interdisciplinary Team and suggesting necessary adjustments.
- Document all nutrition-related activities and notes in participants' medical records in accordance with Senior Community Care standards.
- Provide nutritional counseling and educational programs for participants and caregivers, utilizing appropriate resources.
- Facilitate nutritional education initiatives within the department and across Senior Community Care staff.
