  • The Case Management team at Montrose Memorial Hospital plays a crucial role in guiding patients through their healthcare journey, providing essential information about their treatment options and care pathways.

Role Responsibilities:

  • The Patient Care Transition Specialist operates as an integral member of a multidisciplinary team, serving as an advocate, liaison, and educator for patients at high risk for repeated hospital and emergency department visits. This role is pivotal in facilitating a seamless transition for patients, ensuring they achieve a successful recovery following discharge from the hospital or emergency services.
  • This specialist will offer personalized support to patients, families, and caregivers, assisting in identifying and addressing obstacles that may impede effective and timely medical and psychosocial care, which are critical in preventing hospital readmissions.
  • The Patient Care Transition Specialist will engage with patients from the time of hospital admission and continue support for 30 days post-discharge, including conducting home and community visits as part of their responsibilities.

Candidate Qualifications:

  • Completion of an accredited nursing program with at least an Associate Degree in Nursing.
  • A Bachelor of Science in Nursing (BSN) is preferred.
  • Possession of a current, unrestricted nursing license in the relevant state.
  • A minimum of two years of nursing experience is required.
  • Experience as a nurse navigator or patient educator is highly regarded.
  • Basic Life Support (BLS) certification is mandatory upon hire.
