Position Summary

The Community Services Case Manager will deliver support and guidance to families of children and adults eligible for DDS services, including those on the autism spectrum. The role encompasses providing information and referrals, service navigation, support groups, family training, parent networking, mentoring, facilitating social and recreational events, and transportation based on the families' prioritized needs. The Case Manager will assist families in accessing public benefits, entitlements, and community resources available in their local area, while being responsive to the cultural and linguistic needs of the families served.

Work Schedule

Part-time, 20 hours per week (including some weekends).

Key Responsibilities:
  1. Assist families in securing services that address their priority needs.
  2. Create opportunities and provide support for individuals to make informed decisions, develop skills, enjoy creative social experiences, and promote independence.
  3. Deliver services to families and individuals in accordance with DDS, CARF, and all relevant regulatory standards, as well as Bridgewell policies and procedures.
  4. Support families with medical, clinical, and financial requirements.
  5. Transport individuals as necessary to medical, recreational, and other community activities.
  6. Maintain necessary documentation as per DDS and all applicable regulatory standards.
  7. Help families access community resources and recreational opportunities.
  8. Facilitate the management of flexible spending funds as required.
  9. Conduct data collection and reporting in compliance with DDS and/or Bridgewell requirements.
  10. Maintain core training certifications as mandated by DDS and Bridgewell.
  11. Perform other duties as assigned.
Qualifications:Education and Experience:
  1. Bachelor's Degree in a related field or, alternatively, three years of experience in the Human Services sector.
  2. A minimum of one year of experience in the human services field and/or experience working with families of children or adults with intellectual and developmental disabilities, including families of adults on the autism spectrum.
Skills and Knowledge:
  1. Exceptional written and verbal communication skills.
  2. Strong organizational and problem-solving abilities.
  3. Valid driver's license and at least one year of driving experience.
  4. Access to a reliable vehicle with the necessary insurance coverage.
  5. Basic computer proficiency.
  6. Bilingual/bicultural proficiency in Spanish is required.
Physical Requirements:
  1. Frequent climbing of stairs is required.
  2. Ability to lift up to 20 pounds.
  3. Must be able to operate a vehicle.
  4. Normal range of eyesight with or without corrective lenses.
  5. Normal range of hearing with or without assistive devices.
