Job Summary:The RN Case Manager will be responsible for collaborating with the healthcare team to manage the progression of patients' care, utilizing evidence-based practice guidelines. This includes engaging patients and care teams in the joint management of key patient outcomes, such as disease progression, length of stay, and transition to the most appropriate post-acute setting.
Responsibilities:- Collaborate with the healthcare team to develop and implement individualized care plans
- Conduct thorough assessments of patients' needs and develop plans to address those needs
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
- Provide education and support to patients and families to promote self-management and independence
- Current and valid NYS RN license
- Bachelor of Science in nursing or willingness to pursue a defined plan for matriculation in a program
- 2 years of experience in an acute setting
- Preferred requirements: 1+ year of experience in case management or utilization management, Case Management certification (CCM) or Certified Professional in Healthcare Management (CPHM)

- Thorough orientation and continual development from senior leadership
$44.95 per hour, based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
