About the Role:

The Hospice Case Manager (RN) is a key member of our clinical team at Intrepid USA Health Care Services. As a Registered Nurse, you will be responsible for delivering high-quality nursing care to our patients in accordance with their established plan of care.

Responsibilities:

  • Perform comprehensive subjective and objective assessments of patient status, including physical, psychosocial, and environmental parameters.
  • Ensure compliance with federal and state regulations governing hospice care services by following policy and procedure consistent with agency standards.
  • Formulate individualized plans of care that incorporate analysis of assessment data, modifying and updating as needed at a minimum of every two weeks on assigned patients.
  • Provide skilled interventions aimed at achieving realistic outcomes within a specified time period, performed within the nursing scope of practice.
  • Assure thorough and accurate documentation of all clinical data, including admission through discharge, physician's orders, skilled nursing visits, and changes in conditions.
  • Consistently demonstrate competency with technical nursing skills according to the legal scope of practice.
  • Implement teaching specific to patient/family needs and evaluate and modify as needed.
  • Perform clinical chart record reviews as requested.
  • Perform on-call responsibilities as assigned.
  • Determine if the patient meets admission criteria and assist in the assignment of qualified personnel.
  • Assist the physician in developing the plan of care and make necessary revisions based on the patient's condition and needs.
  • Prepare clinical progress notes and review documentation on a continual basis to ensure compliance with hospice documentation standards.
  • Ensure that adequate medical supplies and equipment are provided and reordered as necessary on assigned patients.
  • Adhere to the organization's patient care policies, including the implementation of patient care plans.
  • Perform on-site supervisory and teaching visits for the evaluation of professional and paraprofessional staff on assigned cases and provide follow-up information to the Agency Management as requested.
  • Provide orientation, training, or in-services to the care team to assure the quality and appropriateness of care provided.
  • Assure that documentation by HHA is complete and appropriate to the services provided.
  • Assure compliance with the requirements of state licensure and Intrepid policy by following agency protocols.
  • Recommend new policies and/or changes in current policies to improve the quality of patient services as well as efficiency and effectiveness of services.
  • Complete other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company.

Requirements:

  • Current RN licensure in the state of practice.
  • Minimum of 2 years of experience in hospice care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Proficiency in electronic medical records (EMRs) and other healthcare software.
