Concho Hearts Hospice is seeking a dedicated Licensed Medical Social Worker (LMSW or LBSW) to provide compassionate end-of-life care in the Beaumont area.

Our Licensed Medical Social Workers play a vital role in supporting patients and their families during challenging times. You will utilize your expertise to assess psychosocial needs, develop individualized care plans, and offer guidance and counseling to enhance coping mechanisms and overall well-being.

Responsibilities:
  • Conduct thorough assessments of patients' and families' psychosocial needs related to terminal illness and home environment.
  • Evaluate the impact of medical and nursing requirements on patients' homes, financial resources, and access to community support systems.
  • Collaborate with the interdisciplinary team to develop personalized care plans that address the unique needs of each patient and family.
  • Provide individual and group counseling sessions to patients and families, addressing stress, grief, and other emotional challenges.
  • Maintain clear communication with the RN Case Manager and other healthcare professionals involved in the patient's care.
Qualifications:
  • Master of Social Work (MSW) or Bachelor of Social Work (BSW) from an accredited institution.
  • Current, unencumbered license as a Licensed Medical Social Worker (LMSW) in Texas.
  • Minimum one year of experience as a social worker in long-term care or medical-surgical/acute care settings.
  • Proficiency in electronic medical records (EMR) systems and computer applications.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team in a compassionate and supportive environment.
