Our team of dedicated professionals, including nurses, aides, chaplains, social workers, and doctors, work together to provide symptom management and support to patients and their families during the end-of-life journey.
Job SummaryThe primary function of this role is to administer nursing care to patients of all ages in their place of residence, while coordinating care with the interdisciplinary team, patient, family, and referring agency. This position requires a minimum of 2 days per week, with potential to become full-time.
Responsibilities- Develop and Adjust Patient Care Plans: Prepare and adjust patient care plans as needed to ensure the best possible outcomes.
- Provide Patient and Family Support: Offer loved ones and primary caregivers patient care information, end-of-life education, and emotional support to help them navigate the care process.
- Maintain Accurate Records: Maintain and update medical charts, patient files, and health records to ensure accurate and timely information.
- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Licensure: Current license as a registered nurse (RN) and/or by the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC), current Drivers License.
- Experience: Two years of experience as a registered nurse in a clinical care setting is preferred, with hospice experience being a plus.
- Skills: Nursing skills are defined as generally accepted standards of practice. Good interpersonal skills are essential, as well as specialized skills in palliation and end-of-life care. Proof of current CPR is required, and the ability to read, write, and comprehend English is necessary.
- Transportation: Reliable transportation is required, with a valid and current auto liability insurance policy.
- Environmental and Working Conditions: This role requires working in patients' homes in various conditions, with possible exposure to blood and bodily fluids and infectious diseases. The ability to work a flexible schedule, travel locally, and withstand some exposure to unpleasant weather is necessary. PRN emergency call may be required.
- Physical and Mental Effort: Prolonged standing and walking are required, with the ability to lift up to 50 lbs and move patients. This role also requires working under some stressful conditions to meet deadlines and patient needs, making quick decisions, and acquiring resources as needed to meet patient/family individualized psycho-social needs.
