Job Summary

As a key member of our clinical services team, you will be responsible for providing evidence-based training and support services to individuals and families in need.

Key Responsibilities:
  • Rehabilitation Clinician:
    • Provide skill-building training in the home or community, utilizing evidence-based practices and curriculums.
    • Offer support and crisis/emergency services for individuals and families through face-to-face contact, phone contact, or on-call system.
    • Maintain accurate and timely documentation in electronic records, adhering to regulatory requirements.
    • Establish rapport with clients, family members, and other stakeholders, providing emotional support and encouragement.
    • Collaborate with community partners to ensure clients' needs are met.
  • Case Manager:
    • Conduct clinical screenings, assessments, and service eligibility determinations based on individual needs.
    • Coordinate, link, and authorize appropriate services, monitoring progress towards treatment goals.
    • Deliver interventions such as crisis resolution, admission/discharge planning, and clinical reviews.
    • Participate in staff meetings, training, and professional development activities.
Requirements:
  • Master's Degree in a relevant field.
  • At least one year of paid experience in a related field.
