Job Summary

The Case Manager MSW is a hospital-based professional responsible for coordinating and evaluating care options for patients. This role requires assessing patient needs, partnering with the multidisciplinary team and patient/family to develop a plan that aligns with patient goals and access to resources. The Case Manager coordinates, plans, and delivers interventions related to patients' biopsychosocial needs, while working with a multidisciplinary team and providing expertise related to the field while balancing regulatory guidelines related to Utilization Review and Conditions of Participation for Discharge Planning.

Key Responsibilities
  • Stay current with provider aspects of federal/state organization regulations to ensure compliance with guidelines and report/tracks internal metrics accordingly.
  • Support patients, families, and caregivers to understand, accept, and follow medical recommendations and provide services planned to assist in achieving the optimum social, spiritual, and health adjustment within their capacity and level of health literacy.
  • Display competence in motivational interviewing, social competence, and promote empowerment among a population of hospitalized individuals.
  • Evaluate current treatment plans to identify barriers, determine realistic goals and objectives, and seek potential alternatives in conjunction with the medical staff.
  • Coordinate team efforts with support service departments to ensure appropriate care and smooth discharge transitions.
Requirements
  • Education: Masters degree in Social Services
  • Certification: Certified Social Work (CSW) - South Dakota Department of Social Services
  • Experience: 3+ years of Healthcare Experience
