Job Summary

Monument Health is seeking a highly skilled Lead Case Manager to join our team. As a Lead Case Manager, you will be responsible for providing clinically based care management to support the delivery of effective and efficient patient and family centered care in the acute care hospital setting.

Key Responsibilities:
  • Collaborate with medical staff, patients, family, and the healthcare team to assess, facilitate, plan, and advocate for health needs on an individual basis.
  • Determine realistic goals and coordinates efforts to ensure smooth discharge planning and clear follow-up after discharge.
  • Utilize InterQual criteria guidelines to confirm medical necessity for admission and continued stay.
  • Establish goals and priorities consistent with the mission and goals of Monument Health, as well as meet requirements of JCAHO and other applicable federal, state, and local regulatory and/or accrediting bodies.
  • Collaborate with medical staff and the interdisciplinary team to promote quality patient outcomes.
Requirements:
  • Education: Completion of a nursing education program approved by a board of nursing.
  • Certification: Registered Nurse (RN) - South Dakota Board of Nursing.
  • Preferred: Bachelors in Nursing, 5+ years of Acute Experience, 5+ years of Registered Nurse Experience, Commission for Case Management Certification (CCMC).
