Job Summary

The Orthopedic Care Navigator plays a crucial role in facilitating the care of patients and the progress of the Orthopedic Total Joint Program. This position is responsible for coordinating the flow of patients and progress of the department, ensuring an optimum level of patient care and staff development.

Key Responsibilities:
  • Conduct educational programs and monitor their effectiveness in promoting patient understanding and engagement.
  • Coordinate patient processes from pre-op testing to post-op care, serving as a liaison between physician's offices, the hospital, and other facilities.
  • Develop and implement multidisciplinary orthopedic care pathways, monitoring compliance and revising as needed based on patient satisfaction, standards of care, and quality information.
  • Anticipate provider needs by preparing patients for surgery and assessing, setting goals, preparing, implementing, documenting, and adjusting treatment plans.
  • Market the orthopedic program through community outreach, utilizing patient ambassadors, and providing evidence-based feedback to orthopedic surgeons and providers.
  • Monitor joint replacement clinical outcomes, identify opportunities for improvement, and track program growth and fiscal impact.
  • Monitor patient satisfaction through surveys and discharge phone calls, gathering information on perceptions of care, patient outcomes, and the joint replacement experience.
  • Organize and lead the Orthopedic Program and Joint Center of Excellence, collaborating with all disciplines to drive solutions for identified problems.
Requirements:
  • Education: Bachelor's degree in Nursing
  • Certification: Registered Nurse (RN) - South Dakota Board of Nursing
  • Preferred: 1+ years of Healthcare Experience, 1+ years of Orthopedic Experience, 1+ years of Registered Nurse Experience
