Job Summary

At Monument Health, we are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in ensuring the seamless transition of patients across the continuum of care. Your expertise will be instrumental in coordinating services, facilitating communication between healthcare providers, and advocating for the needs of our patients and their families.

Key Responsibilities:
  • Coordinate patient care transitions, ensuring a smooth and efficient transfer of services.
  • Facilitate communication between healthcare providers, patients, and families to ensure informed decision-making.
  • Assess patient needs and develop individualized plans of care to achieve optimal outcomes.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and effective resource allocation.
  • Identify and address barriers to care, developing creative solutions to ensure patient access to necessary services.

Requirements:

Education: Completion of a nursing education program approved by a board of nursing.
Certification: Registered Nurse (RN) - South Dakota Board of Nursing.
Preferred: Bachelors in Nursing, 5+ years of acute experience, and Commission for Case Management Certification (CCMC).
