About the Role

We are seeking a highly skilled and compassionate Lung Transplant Nurse Coordinator to join our team at Mass General Brigham. As a key member of our multidisciplinary team, you will be responsible for providing exceptional patient care and coordination of services for patients undergoing lung transplantation.

Key Responsibilities
  • Coordinate patient care and services, including scheduling appointments, testing, and follow-up care.
  • Collaborate with the transplant team, including physicians, nurses, and other healthcare professionals, to ensure seamless care coordination.
  • Provide education and support to patients and their families, addressing their physical, emotional, and psychosocial needs.
  • Develop and implement individualized care plans, taking into account the unique needs and circumstances of each patient.
  • Participate in multidisciplinary team meetings and contribute to the development of policies and procedures.
  • Stay up-to-date with the latest advances in lung transplantation and related care, applying this knowledge to improve patient outcomes.
Requirements
  • Graduate of an approved school of nursing with current registration in Massachusetts.
  • Minimum 2 years of experience in pulmonary, thoracic surgery, or transplant nursing, with a strong background in patient care coordination.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
  • Knowledge of community resources and post-acute care providers, with the ability to connect patients with necessary services.
