Clinical Social Worker

Vivo HealthStaff is seeking a highly skilled Clinical Social Worker to join our team in Kentucky. As a 100% remote position, this role offers flexibility and autonomy while working with patients of varying levels of need.
About the Role
This position requires management of a panel of patients, coordinating care between patients, healthcare providers, primary care physicians, and specialty mental health services. The successful candidate will perform psychosocial needs assessments, develop treatment plans, deliver case management, care coordination, therapy, and counseling services.
Key Responsibilities
- Care Coordination: Coordinate patient care between multiple healthcare providers, document all encounters in the Electronic Health Record (EHR), and provide timely updates to the multi-disciplinary team.
- Case Management: Research and facilitate referrals to community services and additional treatment programs based on patient need.
- Clinical Support: Provide brief individual and/or group-based treatment using evidence-based interventions such as Motivational Interviewing, Behavioral Activation, Problem-Solving Treatment, and Cognitive Behavioral Therapy.
Requirements
To be successful in this role, you must possess:
- A Master's Degree in Social Work from an accredited graduate-level program.
- Fluency in Motivational Interviewing, Behavioral Activation, Problem-Solving Treatment, and Cognitive Behavioral Therapy.
- A minimum of 1 year of experience providing direct care for individuals diagnosed with addiction.
- Flexibility to work early evening hours (up to 7pm schedule).
Estimated Salary: $60,000 - $80,000 per annum
