The Brook Hospital Dupont has been providing quality mental health and addiction treatment services since 1985. Our hospital offers a full continuum of care for children and adults, including inpatient, extended care, partial hospitalization, and outpatient services.
About the Role
We are seeking a dedicated and compassionate PRN Weekend Social Worker / Therapist to join our multidisciplinary treatment team. As a PRN Weekend Social Worker / Therapist, you will play a vital role in providing psycho-social assessments, conducting family and group therapy sessions, and coordinating discharge planning.
Responsibilities
- Conduct family and group therapy sessions on assigned cases, obtaining psychosocial assessment information, referral source contact, case coordination, and discharge planning.
- Formulate a preliminary discharge plan with the treatment team and family, ensuring consistency with the goals and philosophies of the program.
- Document discharge planning weekly, completing and signing the Discharge Contract, and maintaining accurate clinical therapy notes.
- Serve as Case Manager for assigned cases, ensuring timely completion of treatment plans and required clinical services objectives.
- Complete clinical service requirements on Treatment Planning forms.

Requirements
- Master's Degree from an accredited school in the Behavioral Sciences.
- Licensed or under supervision in the state of Kentucky (CSW/LCSW CADC/LCADC, MFTA, LMFT, LPCA, LPCC, LPA, LPP) preferred.
- One year of inpatient behavioral health work experience preferred.
