Job Summary

ArchCare is seeking a skilled Social Work Coordinator to join our team. As a Social Work Coordinator, you will develop and maintain therapeutic relationships with participants, optimize their functioning, and provide quality services in conjunction with the interdisciplinary team.

Responsibilities

* Develop individualized life plans with participants, identifying their goals and implementing interventions to achieve them.
* Monitor effectiveness and outcomes regularly, keeping the team informed of participant progress and level of need.
* Document changes in participant condition in the medical record, consistent with documentation policies established by the medical director.
* Manage social work planning for a caseload of participants, maintaining productivity to meet program goals.

Requirements

* 1 year of relevant experience
* Bilingual in Spanish
* High School Diploma
