Medical Care Coordinator

At Lutheran Social Services of New York, we are seeking a highly skilled Medical Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for providing medical care and support to our clients.
Key Responsibilities:- Provide medical care and support to clients, including health screenings, medication management, and triaging medical complaints.
- Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination.
- Develop and implement health policies and procedures to promote the health and well-being of our clients.
- Provide education and training to staff and clients on health-related topics.
- Assist in the development of emergency response plans and ensure compliance with OSHA regulations.
- Registered Nurse (RN) license in New York State.
- Minimum of 2 years of experience as a school nurse or similar experience in a medical or teaching hospital.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Knowledge of NYS and NYC Department of Health regulations.
