As a Case Aide - Social Services Specialist, you will play a vital role in our mission by providing direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries, and follow-up.

  • Manage a diverse client caseload, ensuring timely and effective support.
  • Monitor and document client progress toward service plan goals, utilizing data to inform decision-making.
  • Collaborate with internal and external stakeholders to coordinate client services, including social services, mental health, medical, employment, educational, and child care providers.
  • Conduct regular case conferences with clients and service providers to ensure seamless service delivery.
  • Prepare and submit program reports on a monthly basis, highlighting key achievements and areas for improvement.

Qualifications:

  • Bachelor's degree in social work or a related field; Associate's degree accepted with relevant experience.
  • Two years of case management, social services, or relevant work experience, preferably with elderly populations.
  • Strong communication, time management, and organizational skills, with proficiency in Microsoft Office suite and other standard business technology.

Physical Requirements:

  • Ability to use a computer for prolonged periods.
  • Occasional lifting and/or moving up to 10 pounds.
  • Ability to stand, walk, or sit for long periods, with occasional climbing of five flights of stairs for home visits and fieldwork.
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
