Job Title: Case Manager

CAMBA is seeking a skilled Case Manager to join our team in the Scattered Site Housing Program. As a Case Manager, you will be responsible for providing comprehensive support services to clients with HIV/AIDS, including case management, housing maintenance, and referrals to community resources.

Key Responsibilities:
  • Conduct intakes, assessments, and service plans to identify client needs and goals
  • Provide case management services, including accompaniment, referrals, and advocacy
  • Ensure clients are connected to and maintain primary medical care and adhere to medical and medication treatment
  • Monitor clients' progress toward their goals and document via progress notes
  • Function as client liaison/client advocate with outside organizations
  • Escort clients to appointments and assist with completing applications for benefits and entitlements
  • Collect and maintain client files and conduct regular home visits
Requirements:
  • Bachelor's degree or associate's degree/high school diploma/GED with at least 4 years' experience in case management or a related social service field
  • Two years of relevant experience, preferably in a mid-sized organization working with persons with histories of homelessness, mental illness, and/or substance abuse
  • Bi-lingual preferred
