Senior Case Manager

We are seeking a highly skilled Senior Case Manager to oversee our Case Management team at Services for the UnderServed. The ideal candidate will have a Bachelor's degree in a Human Service field and at least 1 year of supervisory experience. This role requires the ability to work independently and as part of a team, providing guidance and oversight to Case Managers in developing initial service plans and service plan reviews. The Senior Case Manager will also provide training to Case Managers in assisting clients with financial entitlement and medical benefit applications.
Responsibilities
- Provide supervisory oversight and guidance to Case Managers in developing initial service plans and service plan reviews
- Refer clients for in-house and community-based medical, mental health, and supportive social services
- Provide training to Case Managers in assisting clients with financial entitlement and medical benefit applications
- Schedule and assign groups and activities to Case Managers as needed
- Manage program census and census data
- Perform full intake and assessment with new clients within 24 hours of admission as needed
Qualifications
- Bachelor's degree in a Human Service field required
- 1+ years supervisory experience
- Experience working with chronically homeless individuals or families
- Ability to work independently and as part of a team
- Bilingual preferred
- Valid NYS Driver's license preferred
