Key Responsibilities

  • Assess patient needs, develop and implement individualized care plans, and evaluate patient progress.
  • Administer medications, perform treatments, and provide emotional support to patients and their families.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
Requirements
  • Current Washington state LPN licensure.
  • National Provider BLS certification within 30 days of hire.
  • 1 year of experience in long-term care or a related field (preferred).
