The Registered Nurse (RN) will provide care and services to a group of patients, visiting them in their homes, skilled nursing facilities, or assisted living facilities. As a contributing member of our interdisciplinary team, you'll provide expertise on psych-social assessments, interventions, and community resources. You'll work in either a Case Management, Admissions, or Resource role.
Responsibilities- Provide care and services to patients in their homes or facilities
- Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership to provide comprehensive care
- Work independently to manage visits, documentation, and communication with healthcare professionals
- Manage time effectively to prioritize visits, documentation, and meetings
- Learn and utilize an electronic medical records system
- Travel across the service area using reliable transportation
- Current Registered Nurse (RN) licensure in good standing
- Ability to work well in a team environment
- Strong communication and time management skills
- Ability to learn and adapt to new systems and processes
