As a Social Worker at Suncrest Hospice, you'll play a vital role in helping patients and families navigate the complexities of end-of-life care. Your expertise will be invaluable in assessing, diagnosing, and treating mental and social conditions, as well as providing counseling and support.

**Key Responsibilities:**

* Assess the psychosocial status of clients and identify social and emotional factors related to their illness
* Provide direct social casework services, including goal setting, counseling, and planning
* Advise agency personnel, clients, and families on community resources and eligibility criteria
* Participate in the development and renewal of client care plans
* Maintain accurate and timely client care records

**Requirements:**

* Master's degree in Social Work required
