Job Summary

The Registered Nurse (RN) will provide care and services for a group of patients, visiting them in their homes, skilled nursing facilities, and assisted living facilities. As a contributing member of the interdisciplinary team, the RN will provide expertise on psych-social assessments, interventions, and community resources. This role can be in Case Management, Admissions, or Resource.

Qualifications
  • Current Registered Nurse (RN) licensure in good standing in the state(s) of practice
  • Ability to work effectively in a team environment with nurses, social workers, chaplains, volunteers, and CNA leadership
  • Strong communication and time management skills to manage visits, documentation, meetings, and urgent needs
  • Ability to learn an electronic medical records system
  • Reliable transportation, valid driver's license, and auto insurance to travel across the service area
