The Registered Nurse (RN) will provide care and services to a group of patients, visiting them in their homes, skilled nursing facilities, and assisted living facilities. As a contributing member of our interdisciplinary team, you'll provide expertise on psych-social assessments, interventions, and community resources. You'll work in either a Case Management, Admissions, or Resource role.
Qualifications- Current Registered Nurse (RN) licensure in good standing in the state(s) of practice
- Ability to work effectively in a team environment with nurses, social workers, chaplains, volunteers, and CNA leadership
- Strong communication and time management skills to manage visits, documentation, and meetings
- Ability to learn an electronic medical records system
- Reliable transportation, valid driver's license, and auto insurance to travel across the service area
