Key Responsibilities:

  • Facilitating the creation, development, and execution of personalized support plans by conducting assessments, evaluations, and ongoing reviews of data, ensuring progress towards defined goals.
  • Assisting individuals with Intellectual Disabilities in accessing essential medical, social, educational, and other resources.
  • Delivering or coordinating essential case management activities for adults with Intellectual Disabilities, including:
    • Conducting comprehensive needs assessments to identify strengths, weaknesses, and desires.
    • Participating in the development and implementation of the Plan of Care within specified timelines.
    • Helping individuals and families identify necessary services and connecting them with appropriate resources as outlined in the Plan of Care.
  • Providing social support by helping individuals and families build or expand their social networks through connections with relevant individuals, support groups, and agencies.
  • Conducting regular reassessments and follow-ups to evaluate progress towards objectives, documenting outcomes, and making necessary adjustments to the Plan of Care.
  • Monitoring the adequacy of services provided and ensuring they meet the individual's needs, while maintaining thorough documentation for accountability and billing purposes.
  • Participating in interdisciplinary and interagency meetings as required.
Qualifications:
  • Must be at least 18 years old.
  • Bachelor's degree in a relevant field or equivalent experience.
  • A minimum of four years of experience in case management.
  • Valid driver's license with a satisfactory driving record as defined by AbleLight.
