Key Responsibilities:

  • Assess patient needs, identify opportunities for care improvement, and develop effective care plans.
  • Collaborate with healthcare professionals, including doctors, social workers, and community resources, to deliver comprehensive care.
  • Travel within assigned regions to visit patients in their homes, community centers, hospitals, or providers' offices.

Requirements:

  • Current, unrestricted licensure as a Registered Nurse (R.N.)
  • 2+ years of relevant clinical work experience
  • 1+ year of community case management experience coordinating care for individuals with complex needs

Preferred Qualifications:

  • Behavioral health or clinical degree
  • Experience with electronic charting
  • Experience with arranging community resources

New York Residents Only: The hourly range for New York residents is $28.03 to 54.95 per hour. Pay is based on several factors, including education, work experience, certifications, and more.
