Job Summary:

Ronald McDonald House New York is seeking a highly motivated and dedicated Patient Navigator to join our team through AmeriCorps. As a Patient Navigator, you will play a critical role in building the infrastructure needed to advance health equity outcomes for underserved medically complex children and their families.

Key Responsibilities:

  • Conduct Social Risk Assessments: Identify social determinants of health impacting target communities and develop tools for data collection and evaluation.
  • Build Partnerships: Establish strong relationships with community-based organizations to provide essential services to patients.
  • Link Patients to Services: Connect patients with community resources, including mental health, housing, food assistance, and other supportive services.
  • Conduct Research and Community Canvassing: Gather information on existing community services and build knowledge on recommended organizations to provide social determinants of health services.
  • Present Findings and Recommendations: Share research and community canvassing results with stakeholders and develop strategies for implementation.
  • Establish Referral Relationships: Develop partnerships with community-based organizations to provide social determinants of health services to patients.
  • Manage Interactions and Interventions: Document and manage interactions with patients, community-based organizations, and other stakeholders.

Qualifications and Competencies:

  • Spanish Speaker Preferred: Ability to communicate effectively with patients and community-based organizations.
  • Even-Temperament and Emotional Balance: Ability to work with diverse populations and manage challenging situations.
  • Cultural Sensitivity and Effective Communication: Ability to communicate effectively with patients and community-based organizations.
  • Collaboration and Partnership Building: Ability to work with diverse stakeholders to achieve common goals.
