Overview:

Shaker for ChristianaCare Direct is seeking a dedicated Registered Nurse (RN) Case Manager with expertise in Discharge Planning within an Acute Care Hospital environment. The RN Case Manager will play a crucial role in the coordination of patient care and facilitate the development of effective discharge plans.

Key Responsibilities:

  • Collaborate as part of an interdisciplinary team to create, implement, and enhance discharge planning processes.
  • Conduct comprehensive assessments to identify patients requiring post-acute care and complex discharge planning.
  • Work closely with physicians, nursing staff, and other healthcare professionals to ensure a multidisciplinary approach to patient care.
  • Identify and address system barriers that may impede patient care progression.

Care Management Model:

Our Care Management Triad Team Model emphasizes collaboration among:

  • RN Case Manager: Oversees patient care, drives progression, and formulates discharge plans.
  • Social Worker: Addresses psycho-social challenges and supports discharge requirements.
  • Utilization Management: Evaluates patient status for appropriateness and payer coverage.

Qualifications:

  • Active RN license in DE or Compact State.
  • Bachelor of Science in Nursing (BSN) required.
  • A minimum of 2 years of medical-surgical nursing experience in an acute care setting is preferred.
  • At least 1 year of experience in Care Management is strongly preferred.
  • BLS certification preferred.
  • Case Management Certification (CCM) must be obtained within 18 months of eligibility.
  • Completion of 8 continuing education credits (CEU) annually in Case Management.
