Shaker for ChristianaCare Direct is seeking a dedicated Registered Nurse (RN) Case Manager with expertise in Discharge Planning within an Acute Care Hospital Environment.
The RN Case Manager will play a pivotal role in coordinating patient care and facilitating progression towards a comprehensive discharge plan. This involves collaborating as part of an interdisciplinary team, crafting, executing, and enhancing all facets of discharge planning to ensure effective, timely transitions throughout the continuum of care.
Care Management Framework:
Our Care Management Triad Team Model fosters collaboration among:
- RN Case Manager: Oversees patient care, promotes patient progression, and formulates discharge plans.
- Social Worker: Addresses psycho-social barriers and supports discharge requirements.
- Utilization Management: Evaluates patient status for appropriateness and anticipated payer coverage.
Responsibilities in Care Management:
- Identify patients with complex discharge planning needs through comprehensive assessments that encompass both physical and psycho-social factors.
- Proactively initiate and develop discharge plans for patients requiring post-acute care, collaborating with physicians, nurses, and other healthcare professionals, as well as patients and their families, following established clinical guidelines.
- Review admission assessments and work with primary nurses and healthcare providers to ensure a multidisciplinary care plan is established to meet identified patient needs and desired outcomes.
- Recognize systemic issues that may hinder care and participate in developing and implementing strategies to eliminate barriers and enhance patient progression.

Qualifications:
- Active RN license in DE or Compact State is mandatory.
- BSN is required.
