Overview:

Shaker for ChristianaCare Direct is seeking a dedicated Registered Nurse (RN) Case Manager with expertise in Discharge Planning within an Acute Care Hospital Environment. The RN Case Manager plays a pivotal role in orchestrating patient care and advancing patient progression to formulate an effective discharge strategy.

Key Responsibilities:

  • Function as an integral member of an interdisciplinary team, developing, implementing, and refining all facets of discharge planning to ensure seamless transitions throughout the healthcare continuum.
  • Conduct comprehensive assessments to identify patients requiring post-acute care, placement, and complex discharge planning, considering both physical and psycho-social factors.
  • Collaborate with physicians, nursing staff, and other healthcare professionals to establish a multidisciplinary care plan tailored to meet the identified needs and desired outcomes of patients.
  • Proactively identify systemic barriers to care and engage in the development and execution of strategies aimed at overcoming these obstacles to facilitate patient progression.

Qualifications:

  • Active RN license in DE or Compact State is required.
  • Bachelor of Science in Nursing (BSN) is mandatory.
  • A minimum of 2 years of medical-surgical nursing experience in an acute care setting is preferred.
  • At least 1 year of experience in Care Management is strongly desired.
  • BLS certification is preferred.
  • Case Management Certification (CCM) must be obtained within 18 months of eligibility.
  • Completion of 8 continuing education credits (CEU) annually in Case Management is required.
