Overview:
Shaker for ChristianaCare Direct is seeking a dedicated Registered Nurse (RN) Case Manager with expertise in Discharge Planning within an Acute Care Hospital environment. The RN Case Manager plays a crucial role in coordinating patient care and facilitating the progression towards an effective discharge plan.
Key Responsibilities:
- Function as an integral member of the interdisciplinary team, developing and implementing comprehensive discharge plans to ensure smooth transitions throughout the care continuum.
- Identify patients with complex discharge planning needs through thorough assessments that consider both physical and psycho-social factors.
- Collaborate with physicians, nursing staff, and other healthcare professionals to create a multidisciplinary plan of care tailored to meet patient needs and desired outcomes.
- Address systemic issues that may hinder patient care and contribute to the development of strategies to overcome these barriers.
Care Management Model:
Our Care Management Triad Team Model consists of:
- RN Case Manager: Oversees patient care, drives progression, and formulates discharge plans.
- Social Worker: Addresses psycho-social barriers and supports discharge requirements.
- Utilization Management: Evaluates patient status for appropriateness and anticipated payer coverage.

Qualifications:
- Active RN license in DE or Compact State is required.
- Bachelor of Science in Nursing (BSN) is mandatory.
- A minimum of 2 years of medical-surgical nursing experience in an acute care setting is preferred.
- At least 1 year of experience in Care Management is strongly desired.
- BLS certification is preferred.
- Case Management Certification (CCM) must be obtained within 18 months of eligibility.
- Completion of 8 continuing education credits (CEU) annually in Case Management is required.
