Job Overview

The Client Support Specialist will deliver a wide array of support services aimed at enhancing the well-being and community integration of individuals. This role is essential in ensuring that each participant has access to vital resources and services that foster a fulfilling life within the community.

Key Responsibilities

The Client Support Specialist will:

  • Establish a personalized relationship with clients to evaluate their needs and preferences.
  • Facilitate access to necessary medical and mental health services.
  • Assist clients in securing and maintaining their entitlements.
  • Provide guidance and advocacy concerning housing requirements.
  • Support clients in nurturing family and social connections.
  • Encourage involvement in community activities to combat social isolation and improve quality of life.
  • Offer individual counseling as required.
  • Conduct group sessions to foster peer support.
  • Accompany clients to appointments when necessary.
  • Monitor clients during hospital stays and collaborate with healthcare professionals to ensure safe transitions back to the community.
  • Organize and lead workshops on relevant topics for clients.
  • Provide crisis intervention as needed.
  • Maintain accurate case records in compliance with organizational and regulatory standards.
  • Ensure adherence to all confidentiality policies.
  • Contribute to the management of client data systems as required.
Additional Duties
  • Exhibit a high level of respect and care for clients.
  • Work collaboratively with team members.
  • Participate in training and professional development opportunities.
  • Engage in community outreach and collaboration with other organizations.
  • Attend regular supervision and team meetings.
  • Perform other responsibilities as assigned.
Qualifications
  • Bachelor's degree in Social Work (BSW) is considered; Master's degree in Social Work (MSW) or Licensed Master Social Worker (LMSW) is preferred.
  • Experience working with elderly populations or individuals with special needs is advantageous.
  • Experience in group facilitation is preferred.
  • Bilingual proficiency in English and Spanish is strongly preferred.
